Example (Higher to Lower) when Bolus is > 99 hours

1. Determine the bolus dose.
Bolus Dose = 0.47 ml x 500 µg/ml = 235 µg
2. Determine the bolus duration.
Bolus Duration = 2000 µg/ml x 0.47 ml = 112.7 hours
3. Using the Simple Continuous Infusion Mode, enter the reservoir volume
and new drug rate on the Changes Screen.
Leave the old drug concentration entered; do not enter the

4. Verify the old drug concentration, reservoir volume and desired drug rate
are programmed on the Pump Status After Update Screen.
5. Perform a reservoir rinse twice and refill the pump with the new
concentration.
6. Schedule patient for reprogramming based on the bolus duration calculated
in Step 2.
CAUTION: When the old drug is through the catheter, the patient will
begin to receive the new lower drug concentration at the slow
rate of the previous concentration. The patient needs to be
reprogrammed to prevent being underdosed; however, never
reprogram the pump early; this could result in clinically
significant or fatal drug overdose.
7. After approximately 5 days, enter the new drug concentration and changes
in daily dose; update the pump via telemetry.
8. Verify the new prescription on the Pump Status After Update Screen.
CAUTION: There is a residual volume in the reservoir that cannot be
removed by emptying the pump. Therefore, to prevent drug
overdose when changing from a higher to lower concentration
or when changing solutions, a reservoir rinse must be
performed. If the reservoir is not rinsed, the actual
concentration is greater than intended and can result in a
clinically significant or fatal drug overdose. Refer to Appendix
B: Performing a Reservoir Rinse.
