Calculating a Bridge Bolus

Whenever you change concentrations or solutions in the pump reservoir, the
remaining old drug in the pump tubing and catheter must be accounted for to
prevent under- or overdosing the patient while the old drug concentration is
being cleared from the fluid pathway. The following methods offer guidelines to
change concentrations and solutions.
CAUTION: Use care when calculating and programming a bridge bolus.
Six values are needed to calculate the Bridge Bolus amount and duration: the
pump tubing volume, catheter volume, their sum, old drug concentration, new
drug concentration, and the desired hourly drug rate.
Determining the Pump Tubing Volume
The volume in the pump tubing from the reservoir to the tip of the catheter port
differs for different pumps.
Depending on the change in concentration the pump tubing values are illustrated
in Table 8.
To prevent under- and overdosing, use the minimum values when increasing
concentrations of drug in the reservoir and the maximum values when
decreasing concentrations. Refer to Table 8 for these values.

1. Identify pump model.
2. Determine whether the concentration is increasing or decreasing.
3. Use Table 8 to determine the appropriate pump tubing volume.
