Description
The 10004S is a "Y" adapter with the addition of a three way stopcock on the short leg to toggle between
cardioplegia and venting. The inlet begins with a female luer fitting. The venting limb terminates in a 1/4" (0.64 cm)
slip connector. Both perfusion legs terminate with locking male luers.
The short leg of the #10005 and #10005OS terminates with a locking male luer fitting. One of the outlet arms of
the adapter terminates with a tubing slip connector that connects to a 3/16" (0.48 cm) to 1/4" (0.64 cm) I.D. vent
line. The other arm terminates with a locking female luer fitting to connect with the male luer of the administration
set. Color coded clamps on the #10005 and clamps with color coded arrows on the #10005OS control the direction
of flow.
The #10005BH is a combination of Medtronic DLP's #10005 and #10004. This double "Y" configuration terminates
in two locking male luer fittings on one side (like the #10004) and a slip connector and locking female luer (like the
#10005) on the other side.
The #10005S is a four legged perfusion adapter. The inlet begins with a locking female luer fitting attached to
3/16" (0.48 cm) tubing. Through a series of "Y" connectors the adapter splits into four legs two of which terminate
with soft silicone vein graft cannulae, the third terminates with a locking male luer, and the fourth terminates with
a slip connector that is used to vent the system.
Sterile, Non-pyrogenic, Disposable.
Indications
This adapter is intended for use in conjunction with cardiopulmonary bypass surgery.
Contraindications
This device is not intended for use except as indicated above.

None known.
Directions for Use
1.
Inspect the package and product for damage and expiration date. If undamaged and unexpired, open the
package and transfer the product onto the sterile field utilizing an aseptic technique.
2.
Connect the tubing slip connector to the vent line and the female luer to the administration set.
3.
Prime all tubing and the vein graft cannulae (#10005S only) with cardioplegia fluid.
4.
To connect the tubing to the cardioplegia cannula, allow the cardioplegia fluid to drip from the end and
connect the male luer adapter to the inlet fitting of the preprimed cannula.
5.
To connect the vein graft cannula (#10005S only) to the bypass graft, first prime the graft retrogradely taking
care to expel all of the air. Open the clamp and allow the cardioplegia fluid to drip from the end of the vein
graft cannula. Insert the cannula loosely into the graft. Stop the retrograde flow. Carefully insert the vein graft
cannula into the graft firmly. While holding it into place, perfuse the graft antegrade.
6.
Upon completion of the procedure, disconnect the adapter and dispose of properly according to hospital
policy with the other blood contaminated products.
