Note: If the procedure is not done under general anesthesia, administer the
appropriate sedation to reduce patient discomfort during the tunneling portion
of this procedure.
1. Identify the tunneling path. Tunnel from the lead site to an intermediate
incision on either the top portion of the parietal bone or the mastoid
area, to the neurostimulator pocket.
2. Make a secondary pocket at the intermediate incision site to contain the
lead-extension so that the lead-extension connection does not lie
directly below the incision.
3. Refer to the tunneling tool manual for detailed instructions on how to
assemble the tunneling tool and perform the tunneling procedure.

Connecting the lead to the extension

Cautions:
▪
Do not use saline or other ionic fluids at connections, which could
result in a short circuit.
▪
Before connecting components, wipe off any body fluids and dry all
connections. Fluids in the connection may result in stimulation at the
connection site, intermittent stimulation, or loss of stimulation.
Two different boot styles are provided, winged and cylindrical.
For each lead:
1. Slide the appropriate boot(s) over the proximal end of the lead(s):

Push the clear boot over the proximal end of the lead in a single-lead
system or over the first lead (left hemisphere) in a dual-lead system
(Figure 10A).

Push the white radiopaque boot over the proximal end of the second
lead (right hemisphere) in a dual-lead system (Figure 10B).
Note: For dual-lead systems, the clear boot will distinguish the first lead-
extension (left hemisphere) from the white radiopaque boot over the
second lead-extension (right hemisphere).

2. Wipe the lead body and the extension setscrew connector junction with
sterile gauze. If necessary, use sterile (United States Pharmacopeia
[USP]) water or a nonionic antibiotic solution to rinse the connections.
After rinsing, dry all connections.
3. Hold the extension securely in your fingers. Insert the lead fully into the
extension setscrew connector (Figure 11).
