Training procedure and follow-up
5 MEASURING OBJECTIVES AND PROCEDURES
It would be best to perform the following tests (fluoroscopy, ENMG, ventilatory
mechanics), or x-ray or ultrasound simultaneously during one session. At least they
should be performed during the same day of the conditioning period. This would
provide data for correlation of the different methods, which would allow to use the
easiest available method for follow- up and trouble shooting later during clinical
course. Also, data would be available to compare the results of different investigators.
All tests should be performed with the patient sitting and in the supine position.
Bilateral and unilateral left and right stimulation should be tested separately.
During the conditioning period, single electrode combinations should be tested also at
certain intervals. Timetables (procedural flow chart 1 and 2) are added which suggest
which tests to perform when.
Fluoroscopy
The standards published by Glenn (Glenn & Phelps 1985) should be followed. An
x-ray positive measure (cm) should be fixed to the under side of the fluorotube. The
distance of the tube to the table should be 12 inches. The patient is in the supine
position.
Ventilatory mechanics
Appropriate studies are performed with the patient sitting and in the supine position.
- Tidal volume curves or peak values should be recorded. TV tests
should also be performed when only one electrode combination is
active at a time. These recordings may be of great value for possible
later trouble shooting.
- The tests should also be recorded during bilateral stimulation at
different levels of "VOLUME" (%) from 10 ... 90 % at steps of 10%.
Also the volume curve or peak value of a sigh should be recorded.
