7.8 The Dura-Guard "R" Series
tip forms a pad of bone at
the bottom of the burr hole.
This pad protects the dura
from the cutting edges and
pushes the dura away from
the skull. After irrigation
and suctioning, the pad is
easily removed with a pick
or elevator type instrument,
such as the ACRA-CUT
Model 800-020. If it is
suspected that the dura
is adhering to the bone,
EXTRA CAUTION must
be taken or dura may be cut.
(See Figure 3)
8.0 WARNING:
Failure to heed the warnings
or to follow the procedures
as described could result in
serious patient injury, death
or product malfunction.
8.1 Surgeon should always be
aware of the possibility
that the perforator may not
disengage.
8.2 Excessive hand pressure
must be avoided to preclude
penetration of the dura and/or
brain. Allow the perforator to
do the cutting.
8.3 Reduce pushing force and
observe caution as the
perforator nears the point of
pentration as nicking, cutting,
or tearing the dura can occur.
8.4 Hold power unit/perforator
perpendicular (90 degrees)

to the skull at the point of
penetration and do not "rock"
to speed cutting action.
Failure to observe may cause
product malfunction, patient
injury or dural lacerations.
8.5 Extreme caution must be
taken when perforating in
areas of the skull that have
1mm or greater variations
in bone thickness. Drill may
nick or cut dura or brain
(similar effect as not being
90 degrees).
8.6 If conditions such as adherent
dura, high intracranial
pressure or other underlying
abnormalities are present in
the area of the penetration,
the perforator may cut the
dura or brain.
8.7 Speed for power drilling
should not exceed 800 RPM.
See Paragraph 7.4 for
procedure.
8.8 Although the perforators are
designed to release safely
and reliably (DGR-I/DGR-0
units in skull/skull areas at
least 3mm thick; DGR-II
units in skull/skull areas as
thin as 1mm) caution must be
observed when perforating
thin skull areas, such as
temporal bone, infants,
children, elderly, or diseased
bone, since skull consistencies
and thicknesses can vary and
the dura could be cut.
