Cause of accidents

The following is an incomplete list of
risks which describes typical hazards
and examples of common causes
of accidents when using ladders.
Take the following risks into consid-
eration as the basis for safe work
when handling the ladder to prevent
accidents.
a. Instability
1. Incorrect positioning of the ladder
(i.e. incorrect inclination angle of
leaning ladders or step ladders
not fully extended);
2. Slipping of the lower ladder end
outwards, i.e. from the wall on
which the ladder is leaning;
3. Lateral sliding of the ladder, falls
to the side and toppling of the
upper tread (i.e. by leaning too
far out or insufficient hold on
the stair);
4. Poor ladder condition (missing
or damaged rubber caps on the
base);
5. Stepping off an unsecured
ladder at a height;
6. Unsuitable surface for the ladder
installation location (i.e. unstable,
soft ground, sloping ground,
slippery surfaces or contaminated
solid surfaces);

7. Bad weather (i.e. wind);
8. Collisions with a ladder, i.e. from
vehicles or doors;
9. Wrong ladder selected (i.e.
too short ladder, improper
application).
b. Improper handling
1. Propping up the ladder in the
working position;
2. Setting up and dismantling
the ladder;
3. Carrying objects when ascend-
ing the ladder.
c. Slipping, tripping and
falling
1. Unsuitable footwear;
2. Contaminated rungs;
3. Careless behaviour on the part of
the user (i.e. taking two steps at
once, sliding down the slide rails).
d. Ladder construction
malfunction
1. Poor ladder condition (i.e. dam-
aged side rails, wear);
2. The ladder has been overloaded.
e. Electrical hazards
1. Essential work involving electric
voltage (i.e. troubleshooting);
2. Setting up the ladder in the
direct vicinity of live electrical
