INTEGRA LIFESCIENCES warrants that each new INTEGRA LIFESCIENCES product is free from defects in material and workmanship under
normal use and service for a period of one (1) year (except as otherwise expressly provided as to accessory items) from the date of delivery by
INTEGRA LIFESCIENCES to the first purchaser, but not beyond the "Expiration" date stated on any product labeling. MAYFIELD surgical devices
are guaranteed to be free from defects in material and workmanship when used normally for their intended purpose. Any covered product that is
returned to INTEGRA LIFESCIENCES for repair or replacement will be carried out at INTEGRA LIFESCIENCES' sole discretion, at INTEGRA
LIFESCIENCES' expense, subject to the terms of this warranty and applicable agreements. Defective products should be returned promptly,
properly packaged and postage prepaid. Loss or damage in return shipment to INTEGRA LIFESCIENCES shall be at CUSTOMER's risk.
IN NO EVENT SHALL INTEGRA LIFESCIENCES BE LIABLE FOR ANY INCIDENTAL, INDIRECT, CONSEQUENTIAL OR PUNITIVE DAMAGES
IN CONNECTION WITH THE ACQUISITION OR USE OF ANY INTEGRA LIFESCIENCES PRODUCT. Further, this warranty shall not apply to,
and INTEGRA LIFESCIENCES shall not be responsible for, any loss arising in connection with the purchase or use of any INTEGRA
LIFESCIENCES product which has been repaired by anyone other than an authorized INTEGRA LIFESCIENCES service representative or altered
in any way so as, in INTEGRA LIFESCIENCES' judgment, to affect its stability or reliability, or which has been subject to misuse, negligence or
accident, or which has been used otherwise than in accordance with the instructions furnished by INTEGRA LIFESCIENCES. THIS LIMITED
WARRANTY IS EXCLUSIVE AND IN LIEU OF ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, AND ALL OTHER OBLIGATIONS OR
LIABILITIES ON INTEGRA LIFESCIENCES' PART, AND INTEGRA LIFESCIENCES NEITHER ASSUMES NOR AUTHORIZES ANY
REPRESENTATIVE OR OTHER PERSON TO ASSUME FOR IT ANY OTHER LIABILITY IN CONNECTION WITH INTEGRA LIFESCIENCES'
PRODUCTS.
INTEGRA LIFESCIENCES DISCLAIMS ALL OTHER WARRANTIES, EXPRESS OR IMPLIED INCLUDING ANY IMPLIED WARRANTY OF
MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE OR APPLICATION OR WARRANTY OF QUALITY, OTHER THAN THOSE
EXPRESSLY SET FORTH IN THE PRODUCT LABELING, INCLUDING THE APPLICABLE USER INFORMATION. The foregoing shall not relieve
INTEGRA LIFESCIENCES from strict tort liability, if otherwise applicable under governing law, for damages for personal injury caused by a product
defect that made the product unreasonably dangerous at the time it was sold or placed.

Descripción
El accesorio de montaje universal para rail lateral
MAYFIELD (REF A-1060) está diseñado para el
acoplamiento del adaptador de barra cruzada
MAYFIELD (REF A-1015 ó A-1016), la mesa de
instrumentos suspendida Garreston (REF A-1053) y el
soporte retráctil Halo BUDDE (REF A-1067) a los railes
laterales de la mesa de operaciones del quirófono
cuando los accesorios de la mesa no son apropiados o
no existen. Se necesitan dos (2) accesorios para rail
lateral.
Inspección
Inspeccione siempre los instrumentos antes y después
de cada uso. Si un instrumento apareciera dañado y/o
pareciera no funcionar adecuadamente, enviar
inmediatamente el instrumento para su reparación o
sustitución. Esto aumentará el tiempo de uso del
producto, así como la seguridad de la vida del
paciente.
ADVERTENCIA: El incumplimiento de las
instrucciones contenidas en este folleto de
productos puede exponer al paciente a lesiones
graves.
Instrucciones de uso
1. Montaje de los accesorios universales para rail
lateral en la mesa de operaciones del quirófano
a. Afloje el tornillo de sujeción girando hacia la
izquierda (Figura 1).
b. Corra los accesorios sobre los railes laterales de la
mesa de operaciones del quirófano.
NOTA: El tornillo de sujeción deberá estar encima de
la superficie plana de los railes laterales de la mesa de
operaciones.
c. Una vez que los accesorios se han corrido hasta la
posición correcta en los railes laterales, apriete el
tornillo de sujecion girando hacia la derecha.
ADVERTENCIA: Antes de usar este producto,
lea atentamente las instrucciones de uso y
advertencias respecto al sistema de apoyo requerido,
contenidas en el folleto del producto.
ADVERTENCIA: Verifique que los accesorios
para rail lateral estén sujetos firmemente a la
mesa de operaciones.
2. Montaje del equipo de apoyo requerido en el
accesorio universal para rail lateral MAYFIELD:
a. Afloje la manivela en T inmovilizadora girando hacia
la izquierda para abrir el receptáculo del rail lateral y
poder montar el equipo de apoyo requerido (Figura 2).
b. Inserte los postes de soporte de dicho equipo en los
receptáculos del rail lateral.
c. Apriete las manivelas en T inmovilizadoras de los
accesorios universales, girando hacia la derecha, para
asegurar los postes de soporte.
ADVERTENCIA: El hecho de no colocar al
paciente en la posición correcta y de no apretar
a fondo todas las piezas de ajuste de este dispositivo o
de otros equipos similares puede exponer al paciente a
lesiones graves.
Limpieza y Esterilización
Después de cada uso, limpie y seque con un paño
concienzudamente el equipo con una solución
antiséptica. Aunque por regla general no es necesario
ni se recomienda, el instrumento puede esterilizarse
con gas, si fuera necesario.
¡ NO ESTERILIZAR EN AUTOCLAVE ¡
Los componentes de plástico se pueden dañar
por el calor.
