The undersigned MORETTI S.p.A Company, with headquarters in Via, Bruxelles 3 -
Meleto, 52022 Cavriglia (Italy), responsible for planning, manufacturing, packaging and la-
belling the technical aids:"Folding chair EM 300", such as those disciplined by the Council's
93/42/EEC Directive, dated June 14,1993, concerning class 1 medical devices subject to the
procedure established in Art. 11 caption 5, of the Directive itself,
GUARANTEES AND DECLARES
that the above-mentioned aids, put in service and on the market starting from the date of this
document, integrally comply with the above-mentioned Directive, which is applicable to the
aids.
