NOTE: It is forbidden to stand on the footrests as this can
lead to a dangerous fall and injury.
ATTENTION
- the wheelchair has a folding frame - it is folded by pulling
the central part of the seat upwards and unfolded by
pressing the sides of the seat downwards. Make sure that
the sides of the seat rest accurately in the plastic hooks.
Failure to unfold the seat accurately can result in the
catches breaking and damaging the wheelchair.
CAUTION:
In the event of a device-related „serious incident" that has
directly or indirectly led to, could have led to or may lead
to any of the following:
(a) death of a patient, user or other person, or
(b) temporary or permanent deterioration of the health of
a patient, user or other person, or
(c) a serious risk to public health
this ‚serious incident' must be notifi ed to the manufacturer
and to the competent authority of the Member State in
which the user or patient is resident. In the case of Poland,
the competent authority is the Offi ce for Registration
of Medicinal Products, Medical Devices and Biocidal
Products.
NOTE:
In case of pain, allergic reactions or other disturbing,
unclear to the user symptoms related to the use of the
medical device, consult a healthcare professional.
ATTENTION: The manufacturer cannot be held responsible
for
damage
caused
inadequate servicing or as a result of not following the
instructions in this manual.
ATTENTION: It is forbidden to use the product other than
in accordance with its intended use!
ATTENTION: During use and handling of the product and
when assembling and adjusting the mechanisms, there
may be a danger of entrapment and/or compression of
parts of the user's/companion's body in openings/gaps
between components. Perform these operations with
particular care.
NOTE: There may be a risk of tipping over if the product is
used incorrectly. Please follow the recommendations for
getting on/out/moving.
Once the adjustment has been completed, stabilise the
positions by tightening the nuts/bolts carefully.
PURPOSE
The wheelchair is intended for the mobility of a disabled
person or a person with mobility diffi culties.

TARGET PATIENT GROUP
Persons suff ering from diseases, dysfunctions or injuries
for the treatment/rehabilitation or compensation of
which this device is intended (see section on intended
use of the device in this instruction manual). The device
can be purchased by the user on their own or on the
recommendation of a doctor, therapist or other specialist.
Whether you purchase the device yourself or on the
recommendation of a doctor/therapist/other specialist,
you must take into account the available sizes/necessary
functions and variants of the device, the indications and
contraindications for use and the information provided by
the manufacturer.
HOW TO DISPOSE OF THE PRODUCT AT END-OF-LIFE
Once the device has been withdrawn from service, the
medical device can be disposed of as normal municipal
waste.
